• Employee Tax Forms  (to be uploaded under Staff Submissions Form if applicable)
• Independent Contractor Tax Form  (to be uploaded under Staff Submissions Form if applicable)
• Completed Staff Submissions Form including the following documents uploaded:

         -Driver’s License

         -Resume

         -Proof of Bachelor’s Degree/s (if applicable)

         -Proof of Master’s Degree/s (if applicable)

         -Proof of Doctorate Degree/s (if applicable)

         -Photo (headshot style) for our website

         -Signed Homework Hub Contract (will be sent to you by email)

         -W-4 Form For Employees

         -IT-2104 Form For Employees

         -I-9 Form For Employees

         -NYS Paid Family Leave Waiver For Employees

         -W-9 Form For Independent Contractors

         -NYS Sexual Harassment Training - Certificate of Completion (if applicable)

• Completion of a background check (an invitation will be sent to you from Checkr by email)
