Parent/Guardian Attendance Requirement

Homework Hub field trips are family-attended experiences. By signing this form, I acknowledge that I (or a designated guardian) will attend the field trip with my child and assume full responsibility for my child’s supervision, safety, and behavior at all times. Homework Hub staff facilitate the group experience but are not responsible for individual supervision.

Assumption of Risk

I understand that participation in off-site activities involves inherent risks, including but not limited to transportation, physical activity, environmental conditions, and interactions in public spaces. I voluntarily assume all risks, both known and unknown, associated with my child’s participation.

Medical Authorization

In the event of an emergency, I authorize Homework Hub staff to obtain necessary medical care for my child, including transportation to a medical facility, if I cannot be reached. I accept full financial responsibility for any medical treatment provided.

Release of Liability

To the fullest extent permitted by New York State law, I hereby release, waive, and hold harmless Homework Hub, its owners, employees, contractors, and affiliates from any and all claims, liabilities, damages, or expenses arising out of or related to my child’s participation in this field trip, except in cases of gross negligence or willful misconduct.

Indemnification

I agree to indemnify and hold harmless Homework Hub from any claims, damages, or liabilities arising from my child’s actions or participation during the field trip.

Behavior Agreement

I understand that students are expected to follow all Homework Hub rules. Failure to comply may result in removal from the activity or future participation.
