• Completed W-9 Form for independent contractors (to be uploaded under Staff Submission Forms)
• NYS Sexual Harassment Training - Certificate of Completion (to be uploaded under Staff Submission Forms if applicable)
• Digitally signed HwH Online Policy
• Digitally signed HwH Home Policy
• Completed Staff Submissions Form including the following documents uploaded:

         -Driver’s License

         -Resume

         -State Certification

         -A photo (headshot style) for our website

         -A brief bio for our website (click on the link for example) https://www.homeworkhubtutoring.com/our-                     instructors

         -Proof of Master’s Degree/s

         -Proof of Doctorate Degree/s) (if applicable)

         -Signed Homework Hub Contract (will be sent to you by email)

         -W-9 Form for independent contractors

         -NYS Sexual Harassment Training -  Certificate of Completion (if applicable)

• Demo Lesson (if applicable)
• Completion of a background check (an invitation will be sent to you from Checkr by email)
